LOYALTY PROGRAM
At Newport Commons Dental Care, we are pleased to offer a patient loyalty program for patients who do not currently have dental coverage. This plan allows our patients to receive optimal dental care while maintaining their oral health.
Dental Services Include
- Professional dental cleanings (2 per benefit year)
- Perio maintenance cleanings (2 per benefit year) covered at 70%
- Radiographs as needed (X-rays)
- Doctor examinations (1-2 per benefit year)
- Diagnosis
- Cosmetic consultation
- 15% discount cash/check on all other services (including cosmetic procedures)
- 5% discount if you pay with major credit

Annual Cost
- $450 first family member (equivalent to $38 per month)
- $350 second family member (equivalent to $29 per month)
- $250 each additional family member (equivalent to $21 per month)
Provisions of the Loyalty Program
- May not be combined with insurance plans, discounts or promotions
- This Loyalty Program is not an insurance plan, but a payment arrangement provided by M.K. Nguyen DDS, APDC and is not transferable to any other dental office or dental specialist
- Payment for services are due at the date of service
- This program is for you and is not transferable
- Benefits are provided for one year from the date the program is purchased
- Must be paid in full when starting the program
- No refunds will be provided for dues of Program Annual Cost under any circumstances, including failure to schedule and maintain appointments
- If Care Credit is used to pay for the program then 0% discount will be given
- If a Credit Card is used to pay for service then only a 5% discount will be given
